How to get started:

  • If you have private insurance, contact your insurance provider at the number listed on the back of your insurance card for options.
  • If you have state funded insurance through a Pre-Paid Medical Assistance Provider, contact your insurance provider at the number listed on the back of your insurance card for options.
  • If you have Medical Assistance, contact a provider to arrange an assessment to determine treatment options.

Please complete the application and provide all requested verifications. If you would prefer to start the application over the phone, please call 763-324-1270.

 

Completed applications and all verifications can be submitted in one of the following ways:

Once your completed application and verifications are received, you will be contacted within 10 days by phone or mail regarding eligibility. If you have not received a response or have any further questions, please call (763)324-1270.
