Community Health Reports

Anoka County Public Health and Environmental Services leads the community through an Assessment and Planning process every three years.  This process involves community members and key partners to identify the main health priorities facing our community and establish a plan for addressing the priority health issues.  Below are the most recent Community Health Assessment Report and the Community Health Improvement Plan.

Adult Health Survey

As part of the recent Community Health Assessment and Planning process, Anoka County developed a survey to better understand the health status and needs of residents.  4,000 randomly selected households were invited to complete the survey, with 857 adults responding (21.4%).  Results helped inform the above Community Health Assessment and Community Health Improvement Plan.
