The Anoka County Sheriff's Office would like to encourage residents to have their medical information available in case of an emergency.
First Responders
When First Responders arrive at a scene, the victim of a medical emergency is not always able to communicate important medical information. Even if you are not alone when the incident occurs, sometimes the people around you don't know the complete medical history or are too upset to relay the information.
Medical Information Kit
On this page, you will find the tools to make your own Medical Information kit. We recommend that you complete these forms and place them in a plastic bag labeled "Medical Information" and secure them at eye-level to the front of your refrigerator.
This way when First Responders arrive, they can take the information and give it to the paramedics who will pass the information along to the doctor. This information will greatly assist those who are trying to help you.
