   Homes licensed by Anoka County may receive credit for reviewing training materials that do not supply a Certificate of Attendance by completing this form.  Once submitted, this form is sent to Anoka County Foster Care staff.  Please be sure to enter your own email address into the box below to receive a copy of this form for your records, you will also need to record the training on your “Record of Training” form.

   Please consult with your Foster Care Social Worker if you have questions whether an activity, book, video or other material will be accepted.

   If you have been licensed by an agency other than Anoka County and would like to utilize the trainings on our website, please consult with your agency as to how they would like to verify completed trainings.  If approved, a printable version of this form is available on our main Training webpage.

   Forms submitted to Anoka County cannot be forwarded to other agencies.
