Death Records
Death Certificate applications can be submitted in a variety of ways.
Deceased 1997 and After
You can purchase a certified death certificate for a death in Minnesota that occurred in 1997, or after, at the Anoka County Government Center Vital Records Office, by mail or online.
Deceased Before 1997
If a death occurred prior to 1997, certificates must be requested through the county where the death occurred. For Anoka County, you may request death certificates at the Government Center Vital Records Office in Anoka, by mail or online.
In person:
- The Vital Records Office is in the Government Center at 2100 Third Ave. Anoka, MN. We have a public service counter on the 1st floor to accept and process applications.
- You can fill out an Online Application before you come to the office, or you can complete an application at the service counter. You will be asked to pay the fees before processing.
- If you complete the application online, you must come to the Vital Records Office in the Government Center within 10 days to complete the process of paying and showing tangible interest which is required.
Online:
- Complete the application and have your signature verified by a Notary Public.
- Visit this pay link to pay for your request and upload your notarized document in PDF format.
- When your payment is verified and documents approved, we will process your request and mail your documents.
By mail:
- Fill out a Minnesota Death Certificate application and have your signature verified by a Notary Public.
- Mail the form with a check for the required fee to the Vital Records Office at 2100 3rd Ave, Anoka, MN 55303
Required Forms
Download the Minnesota Death Certificate application form, print it, and fill it out. You can also pick up the application form at the Government Center Vital Records Office or at any Anoka County License Center. You may also request that the application form be sent to you.
In some cases, you may need someone to fill out the Statement to Identify Certified Birth or Death Certificate Applicant form.
Required Identification
In order to obtain a certified death certificate, you must show that you are an authorized requester for the record. Any one of the following documents qualify as meeting the requirements unless the document has been altered or changed, or there is evidence of fraud on the face of the document.
- Unexpired picture driver's license of a U.S state or territory.
- Unexpired picture identification card of a U.S state or territory.
- Unexpired U.S federal, state, or local government employee picture ID.
- Unexpired FAA aircraft pilot's license.
- Unexpired U.S military picture ID.
- Unexpired U.S. passport.
Required Fees
- $13 First Certified Death Certificate without cause of death (only for records 1997 to present).
- $13 First Certified Death Certificate with cause of death.
- $6 Each additional Death Certificate of the same record issued at the same time.
